Registration Info

2010/2011 Registration Information

Registration is a multi step process.  Please follow the steps below to register a Player for the 2010-2011 season.   If you have any questions, please contact Registrar, Laurie Bean at Registrar@bysahockey.com.  

Step 1 – Each player MUST register with USA Hockey using their website.  Go to the USA Hockey website at http://www.usahockeyregistration.com/ to register your player(s).  There will be a $32 USA Hockey registration fee.  The registration fee is waived for those players born in 2004 or later.  Remember to save the confirmation code; you will need it later in the process.  You can either print the confirmation code or email it to Laurie Bean.  In your e-mail or printed copy, you must have a bar code.  Registering with USA Hockey MUST be complete by Sept. 8, 2010.

Step 2 -  Print and fill out the following forms.  These must be completed and returned to Laurie Bean at registration time.   Any parent unable to sign must have an approved waiver from the BYSA Board of Directors.  
     Consent to Treat
     Player's Code of Conduct
     Parent Awareness 

Step 3 – For Traveling Teams only, you must submit a copy players Birth Certificate.  Please note that this copy will NOT be returned to you.    

Step 4  - The player must now be registered with BYSA Hockey.  There are 3 ways to do this.

1. Attend a Registration session on September 1 or 2 from 6:30 – 8:00 at the BOR.   Please bring your USA Hockey registration confirmation, a copy of the player’s birth certificate, and the completed forms from Step 2 & 3. 

2. Use BYSA Online Registration page from our web site to registration and pay by credit card. Mail the USA Hockey confirmation codes, completed registration forms and birth certificate from Step 2 & 3 to Laurie Bean.   Please note that this credit card purchse only allows payment in full, with this method there is no option to only pay half.   You only need to complete this step if you want to pay online with your credit card.

3. Mail the completed registration forms from Step 2 & 3, the USA Hockey confirmation codes and your check to Laurie Bean.  Half the registration fee is due at registration with the balance due by November 15.. no exceptions.  
 

Step 5 – For Traveling Teams only, a $100 Sponsorship fee and sponsor form is due before your first scheduled game.
